Teams at PATH (People Acting to Help), Inc.

Provides strengths-based case management and wraparound support for youth with behavioral, developmental, and emotional needs. Coordinates assessments, treatment, psychiatric services, crisis planning, family team meetings, school and community visits, continuity of care, discharge planning, advocacy, and access to basic resources. Maintains contact with families and providers, responds to emergencies, provides assigned crisis coverage, completes documentation, and supports family-driven service planning.

Provides executive administrative and secretarial support to the CEO, COO, and CAO. Manages calendars, communications, documents, board and committee meeting materials, retreats, logistics, records, supplies, research, and special projects. Assists with project documentation, budgets, timelines, and reporting while coordinating executive office operations and supporting PATH’s Board of Directors.

Provides training, support, supervision, and community integration assistance to individuals with intellectual disabilities. Coordinates activities, assessments, team meetings, licensing preparation, and monitoring reviews. Assists with personal care, transportation, facility upkeep, investigations, and program documentation. Drives the program van as needed and participates in supervision and staff meetings.
