Teams at PACS

Provide direct nursing care in a skilled nursing facility: administer medications and treatments, monitor residents, document care, report changes, and collaborate with the interdisciplinary team to ensure regulatory compliance and resident well-being.

Provide emotional support, advocacy, and resources to skilled nursing residents and families. Assist with admissions, care and discharge planning, coordinate appointments and community services, collaborate with interdisciplinary teams, maintain social service documentation, participate in care conferences, and support end-of-life and psychosocial needs to enhance residents' quality of life.

Provide and oversee nursing care for residents in a long-term/post-acute setting: assess, monitor, document conditions; administer medications; collaborate with interdisciplinary team; supervise STNAs and LPNs; respond to condition changes and advocate for resident needs.
