PACE North

PACE North is a Program of All-Inclusive Care for the Elderly (PACE) serving adults aged 55 and older in Michigan. It combines primary and specialist medical care, nursing, therapy, nutrition, social support, home-care services, medications, transportation, and adult-day-center activities through individualized care plans. Its mission is to help eligible people who need nursing-home-level care continue living safely in their homes and communities.
