Provide direct patient care including intake, triage, assessments, clinical procedures (IVs, injections, sutures, EKGs), medication/immunization education, documentation in EMR, care coordination, patient education, participate in population health and quality improvement, mentor staff, respond to emergencies, and support clinic coverage (including occasional Saturdays).

Provide direct patient care in a primary care/urgent care setting: triage walk-ins and calls, perform assessments and clinical procedures (IVs, injections, EKGs, wound care), coordinate referrals and follow-up, document in EMR, educate patients, participate in population health and quality improvement, respond to emergencies, and mentor other staff.

Provide direct patient care including intake, triage, assessments, clinical procedures (EKG, IVs, injections, wound care), medication and immunization management, care coordination and referrals, patient education, accurate EMR documentation, participate in quality improvement, respond to emergencies, and mentor other nursing staff to support clinic coverage.

The Community Resource Navigator conducts assessments on social determinants of health, connects community members to resources, collects data, and provides care coordination.

Coordinate and monitor endocrine patient referrals and follow-ups, perform outreach and education, maintain directories of local endocrine resources, refer patients to programs/support groups, collaborate with clinical team, adhere to ambulatory safety protocols, and complete related duties to support patient care.

The Major Gifts Officer will secure major donations by managing a portfolio of donors, developing fundraising strategies, and collaborating with leadership to enhance donor engagement.
