Lead implementation, optimization, and management of telehealth services across the health center. Oversee remote clinicians, design telehealth policies and workflows, integrate EHR/AI solutions, ensure billing and compliance, evaluate program effectiveness with data, maintain a clinical practice (0.5 FTE), and collaborate with IT, finance, quality, and executive teams to align strategy and budgets.

Provides direct nursing care in a walk-in clinic, including triage, patient intake, examinations, procedures, IV insertion and infusions, specimen collection, phlebotomy, referrals, patient education, documentation, and communication of results. Collaborates with providers and clinical staff, maintains safety and confidentiality standards, participates in quality improvement, and may travel between OCHS sites or work remotely as directed.

Provides integrated psychiatric consultation for primary care patients, including diagnosis, treatment planning, medication recommendations, and management of psychiatric conditions. Collaborates with primary care and behavioral health teams, supports diagnostic clarification and care-level decisions, documents clinical services, participates in quality improvement, and delivers evidence-based, patient-centered care.

Provides asynchronous geriatric e-consultations to primary care clinicians managing complex adults aged 65 and older. Delivers concise, evidence-informed guidance on multimorbidity, medications, cognition, falls, frailty, care transitions, and goals of care. Conducts consultations for highly complex cases, documents recommendations, and leads a five-part clinician education series based on the Age-Friendly Health Systems 5Ms Framework. Supports health equity, interdisciplinary collaboration, and improvements in age-friendly primary care.

Provide direct patient care including intake, triage, assessments, clinical procedures (EKG, injections, IVs, dressing changes), medication/immunization management, patient education, care coordination and referrals, documentation in the EMR, participate in population health and quality improvement, respond to emergencies, mentor staff, and occasionally cover Saturday clinic shifts.

Provide direct patient care in an ambulatory/urgent care setting including triage, assessments, clinical procedures (IVs, injections, wound care), medication/immunization education, documentation in EMR, care coordination, preventive screenings, emergency response, and participation in quality improvement and population health initiatives. Collaborate with multidisciplinary teams and mentor other staff.

Provide direct patient care, triage walk-ins and phone calls, perform clinical procedures (EKG, IVs, injections, wound care), coordinate referrals and follow-up, document in EMR, deliver patient education, participate in population health and quality improvement, respond to emergencies, and mentor other nursing staff. May include occasional Saturday coverage.
