Ontario Health atHome

Ontario Health atHome is an Ontario government agency that coordinates in-home and community-based care across the province. It assesses patients’ needs, delivers care, connects people with community services, manages long-term-care placement, and collaborates with hospitals, primary-care providers, and Ontario Health Teams. Its mission is to help patients live safely at home, recover after hospitalization, and access integrated health and social supports.
