Teams at Ohio's Hospice

Provides skilled hospice nursing care to patients in homes and care facilities during extended and weekend hours. Works under RN supervision, follows care plans, addresses patient and family needs, communicates condition changes, documents care, and drives to patient locations. Requires an Ohio LPN license, CPR certification, valid driver’s license, insurance, background screening, and drug screening.

Provides safe, non-emergency transportation and high-quality care to hospice patients. Responsibilities include operating and maintaining medical transport vehicles, completing logs and service documentation, assisting with patient-care activities, supporting quality improvement, and precepting new staff. The full-time Dayton role requires daytime, nighttime, and inclement-weather driving, a valid driver’s license, safe driving record, background check, and drug screening.

Provides non-emergency medical transportation and basic EMT care for hospice patients. Responsibilities include monitoring patients, administering first aid and oxygen, applying splints, maintaining vehicle safety and supplies, documenting care electronically, supporting caregivers, and participating in quality improvement activities.
