Provide direct care and continuity of care for individuals transitioning from a state hospital step-down program to the community. Deliver functional skills training, assist with treatment plans, coordinate community services, support crisis de-escalation, transport or arrange transportation, document care, and collaborate with internal teams and community agencies to support recovery goals.

Manage Living Room program operations and supervise Care and Drop Off Coordinators. Provide crisis intervention, assessments, case management, staffing coordination, training, chart reviews, reporting, community collaboration, and ensure compliance with HHSC and organizational policies while maintaining program budgets and staff oversight.

Manage utilization review and authorization of behavioral health services, determine medical necessity and funding eligibility, ensure compliance with Texas rules and NTBHA policies, supervise providers, collaborate with hospitals and managed care, maintain documentation and performance metrics, and support continuity of care across the NTBHA service area.

Provide direct and indirect nursing care within a State Hospital Step-Down program, perform comprehensive assessments, administer medications (including injections), document clinical status, support crisis response and on-call needs, collaborate with interdisciplinary teams, travel across NTBHA counties for field visits, and use electronic systems and Microsoft Office to maintain records and coordinate care.

Provide trauma-informed, person-centered crisis intervention and disposition planning for Kaufman County residents. Perform screenings, assessments, crisis follow-up, transitional services, case management, coordination with law enforcement, hospitals, and community providers, and support jail assessment program with monthly 12-hour shift.

Supervise remote telehealth crisis Care Coordinators providing telephonic crisis screening, risk assessment, triage, and care coordination; provide clinical consultation, maintain partnerships with law enforcement and community providers, ensure regulatory compliance and documentation, report metrics, and travel across the NTBHA service area as needed.

Serve as a behavioral health liaison to schools and ESCs, assess needs, coordinate prevention/intervention programs, deliver trainings (Mental Health First Aid, grief/trauma), build stakeholder relationships, maintain reporting, and travel across the NTBHA region to support implementation and resource linkage.

Provide direct and indirect nursing care for children in crisis respite, including assessments, medication administration and education, on-call triage, documentation, collaboration with behavioral health teams, travel to multiple sites, and participation in stabilization and transition planning within regulatory standards.

Provide in-person and telehealth crisis assessments, intervention, care coordination, and follow-up for individuals with severe mental illness and substance use disorders. Work with law enforcement, courts, providers, and community partners; complete documentation, reporting metrics, and participate in treatment planning. Requires flexible hours and significant travel within NTBHA counties.
