Dispenses medications, provides patient counseling and medication management, collaborates with healthcare professionals, ensures legal and regulatory compliance, supervises pharmacy staff, and supports patient-centered care in an FQHC setting.

Provides WIC nutrition assessments, counseling, certification, breastfeeding education, food package assignment, referrals, and follow-up for pregnant women, infants, and children. Facilitates nutrition classes and community outreach, maintains compliant documentation, monitors vendors, supports quality improvement and reporting, and collaborates with healthcare and social service providers. Requires participant-centered communication, knowledge of maternal and child nutrition, breastfeeding support, WIC regulations, and cultural humility.

Coordinates case management, hospital transfers, discharge follow-up, and care with primary care providers. Monitors clinical quality measures, supports quality improvement and regulatory compliance, oversees immunization programs, provides clinical triage, and assists with staff competency evaluation. The role shares supervision of Medical Assistants and promotes integrated, patient-centered care across health center sites.
