Teams at National Health Foundation

Provides post-hospital care coordination for unhoused guests transitioning from acute care. Completes needs assessments, care plans, referrals, discharge coordination, housing placement, documentation, and HMIS/CES records. Collaborates with nursing teams, hospitals, case managers, and community partners; conducts check-ins, home visits, crisis interventions, and warm handoffs. Manages a caseload of at least 25 guests while maintaining compliance, practicing harm reduction, and supporting behavioral concerns.

Provides operational leadership and daily oversight for social services in recuperative care programs. Supervises and develops staff, supports complex client cases and discharge planning, reviews documentation, monitors quality and compliance, coordinates with hospitals and community partners, and supports program development. The role serves individuals experiencing homelessness, medical and behavioral health challenges, substance use disorders, and housing instability while helping maintain consistent service standards across locations.

Manage daily operations and staff of a residential care facility, ensure guest services meet standards, coordinate meals and safety procedures, train staff in crisis intervention, oversee inventories, inspections, timecards, and collaborate with leadership on budgets and guest care.
