Builds trusting relationships, conducts virtual and in-person visits, documents care interactions, connects members with community resources, escalates clinical concerns, collaborates with multidisciplinary teams, and supports mothers through calls and messaging.

Provides bilingual English-Spanish navigation, education, advocacy, and social-resource support to expectant and new mothers through pregnancy and postpartum. The role includes virtual and in-person visits, maternal health education, relationship building, care documentation, multidisciplinary coordination, escalation of clinical concerns, and connections to housing, food, transportation, and other community resources.

Manage Nadia Care’s Indiana provider and community partnership territory, developing relationships with health systems, OBGYN practices, hospitals, FQHCs, and outreach organizations. Drive referrals, account growth, partner retention, and community impact through strategic outreach, presentations, events, and performance tracking. Maintain CRM records, report KPIs, map healthcare stakeholders, and collaborate with care delivery, product, technology, and outreach teams. The role requires 50% travel, including field work and occasional evening or weekend events.

Provides bilingual, culturally sensitive support to expectant and new mothers through virtual and in-person education, care navigation, emotional support, and community-resource referrals. Documents interactions, collaborates with multidisciplinary care teams, escalates clinical concerns, and helps address barriers such as food insecurity, transportation, and housing. Serves primarily families in Memphis-area communities, with flexible weekday scheduling, occasional weekends, and light on-call duties.

Support day-to-day marketing operations: edit creative assets, manage Asana project workflows and SOPs, coordinate vendors and giveaways, execute multi-channel campaigns (email/SMS/social/direct mail), maintain email templates and CMS content, run social media and newsletter production, build tracking (UTMs, short links, QR codes), handle inbound group emails, and deliver campaign analytics and reporting.
