Provides peer-delivered support to individuals with behavioral health concerns, substance use issues, emotional challenges, and intellectual or developmental disabilities. Shares coping strategies, mentors consumers, builds trust, and promotes community inclusion, independence, recovery, resiliency, self-esteem, and productivity. Performs peer-related functions under program supervision and helps consumers navigate human services systems.

Provides behavioral health case management for consumers and families, including needs assessment, person-centered planning, service coordination, advocacy, monitoring, therapeutic relationship building, community referrals, crisis support, and potential inpatient or partial-hospitalization prescreening. Master’s-level managers may provide intensive case management and therapeutic interventions. Services support individuals with mental illness, substance-use concerns, and intellectual or developmental disabilities across various age groups.

Provide case management for individuals with intellectual and developmental disabilities: assess needs, develop and monitor person-centered service plans, coordinate services and community resources, advocate for consumers, maintain therapeutic relationships, and provide crisis support and intensive case management as assigned.

Administer and oversee the Self‑Determination program including agreement and budget development, compliance monitoring, coordination with FMS/EOR/MDHHS/PIHP, training and education, quality assurance, data management, and audit preparation to ensure participant choice and Medicaid compliance.

Complete consumer registration and eligibility determinations, manage Medicaid and other benefits applications and renewals, verify and update insurance in the EHR, process payments and reconciliations, provide benefits enrollment assistance and training, perform audits and data analysis to improve eligibility retention, and support providers and consumers with benefit-related questions and documentation.
