As a Psychiatric Mental Health Nurse Practitioner, you will conduct evaluations, prescribe medication, and collaborate with a care team to provide patient-centered care.

The role involves conducting psychiatric evaluations, diagnosing patients, prescribing medications, crisis intervention, and collaborating with care teams while maintaining documentation.

Provide peer-based support using lived recovery experience: assess needs, develop care plans, connect members to community resources, document contacts and care plans, collaborate with care teams and community partners, attend meetings and outreach, and help meet program KPIs to support individuals affected by substance use.

Role is work-from-home with in-community travel to member homes and DCS locations in Tennessee.
