Supports older adults in the PACE program through individualized and group exercise, therapeutic activities, mobility assistance, and activities of daily living. Monitors participant responses, reports progress or concerns to the interdisciplinary care team, and collaborates with therapy staff to promote safety, strength, balance, endurance, independence, and quality of life.

Builds relationships with healthcare providers, referral sources, patients, and families to promote hospice, home health, palliative care, and PACE services. Supports referrals and admissions, explains care options, coordinates transitions, participates in community outreach and educational events, and tracks referral activity. Requires healthcare knowledge, strong communication, local travel, organizational skills, confidentiality, and HIPAA compliance.

Coordinate and manage PACE participant appointments, including medical visits, therapies, transportation, and follow-ups. Communicate with patients, caregivers, and clinical staff, maintain accurate scheduling records in EHRs, verify patient information, handle changes/cancellations, and support efficient clinic operations while ensuring HIPAA compliance and compassionate customer service.

Coordinate onboarding from offer acceptance through orientation, manage new hire paperwork, employee records, benefits administration, and compliance. Support recruitment, policy implementation, employee relations, training, and performance management to foster positive employee experience and engagement.

Provide comprehensive primary and geriatric medical care to PACE participants, perform assessments, manage acute and chronic conditions, develop individualized care plans, lead medication management and care transitions, and collaborate with an interdisciplinary team to support community-based, person-centered care.

Maintain accurate financial records by preparing monthly journal entries, reconciling general ledger accounts, analyzing revenue and expenses, investigating variances, reconciling Medicare/Medicaid/private-pay payments, assisting with claim processing, supporting financial reporting and projects, and collaborating with staff to improve accounting processes and compliance.

Connect older adults with healthcare, financial, and community supports to help them remain at home. Coordinate services with interdisciplinary PACE team, assist with benefits and housing/transportation needs, document participant information, educate families, and provide advocacy and emotional support to improve participants' quality of life.
