Provide comprehensive primary and geriatric medical care to PACE participants, perform assessments, manage acute and chronic conditions, develop individualized care plans, lead medication management and care transitions, and collaborate with an interdisciplinary team to support community-based, person-centered care.

Connect older adults with healthcare, financial, and community resources to support independent living. Coordinate services with interdisciplinary PACE team, assist with benefits, maintain participant records, provide education and emotional support, and advocate to prevent unnecessary nursing home placement.

Coordinate onboarding from offer acceptance through orientation, manage new hire paperwork, employee records, benefits administration, and compliance. Support recruitment, policy implementation, employee relations, training, and performance management to foster positive employee experience and engagement.

Process and ensure accurate, timely payroll while maintaining compliance with federal, state, and organizational regulations. Manage time and attendance records, maintain payroll data, respond to employee payroll questions, support audits and reporting, and collaborate with HR and Finance while protecting confidential information.

Support PACE program operations by organizing participant records, coordinating meetings, assisting with data collection and reporting, maintaining documentation, and supporting quality assurance and compliance projects. Gain exposure to interdisciplinary care coordination, healthcare operations, HIPAA practices, and continuous improvement initiatives while collaborating with clinical and administrative teams.

Lead and assist individualized and group exercise programs to improve strength, balance, endurance, and functional independence for older adults. Assist with activities of daily living and safe transfers, monitor participant responses, report concerns to the interdisciplinary team, and collaborate with therapy staff to support wellness interventions and promote independence.

Provide adaptable administrative support across multiple departments: coordinate schedules, manage communications, prepare documentation and reports, support projects, and handle day-to-day administrative functions to maintain workflow coverage during staffing gaps and peak workloads.
