Coordinates psychiatric care diversion and transitions from nursing facilities or acute care into community settings. Provides mental health, substance use, medication, diabetes, wellness, and psychoeducation; assesses level-of-care needs; connects participants with community resources; collaborates with families, providers, DMHAS, DSS, and discharge teams. Requires statewide travel and maintaining professional relationships with community agencies.

Supervises the HERO housing support program, including staff scheduling, hiring, training, workload distribution, reporting, documentation, and performance management. Supports housing research, referrals, behavioral health resources, crisis intervention, and participant recovery needs. Maintains relationships with funders and community agencies, enforces safety policies, and travels locally and statewide to support programs and participants.

Oversee Stamford transitional housing programs, including administration, staffing, supervision, scheduling, training, budgeting, safety, and compliance. Provide leadership and support to staff and residents, implement recovery-oriented and evidence-based practices, coordinate with funders and outside agencies, respond to crises, and maintain on-call availability. Travel between local programs and meetings may be required, including transporting individuals in a personal vehicle.

Supports organizational performance systems, including clinical databases, EHR workflows, helpdesk requests, data quality checks, reporting, staff scheduling, training, system updates, and accreditation activities. Provides technical assistance and training to staff, monitors system performance and data integrity, supports operational efficiency and reimbursement opportunities, and helps maintain compliance with HIPAA and data protection standards.
