Provide on-site Physical Medicine and Rehabilitation (PM&R) services as a rehab consultant in a skilled nursing facility. Collaborate with interdisciplinary teams (PT/OT/SLP, PCPs) to manage conditions common in post-acute care, track therapy progress, address barriers to therapy, attend multidisciplinary meetings, and support facility staff education.
