Provides Physical Medicine and Rehabilitation services to patients in skilled nursing and post-acute rehabilitation facilities. Responsibilities include assessing and managing rehabilitation barriers, collaborating with physicians and interdisciplinary PT, OT, and SLP teams, monitoring therapy progress, supporting conditions such as stroke and neurological disorders, and participating in care meetings and clinician education.

Provides specialized Physical Medicine and Rehabilitation services to patients in skilled nursing and post-acute rehabilitation facilities. Collaborates with interdisciplinary therapy and primary care teams to manage conditions, pain, spasticity, and other barriers to recovery. Tracks therapy progress, attends multidisciplinary meetings, supports patient independence, and contributes to clinical leadership and education.
