Provide PM&R/physiatry consultative care to SNF/post-acute rehab patients, collaborating with interdisciplinary therapy and PCP teams to optimize function, manage barriers to therapy, attend multidisciplinary meetings, and participate in ongoing education and facility leadership activities.

Provide PM&R/physiatry consultative care at a designated skilled nursing facility, collaborate with interdisciplinary therapy and PCP teams, manage rehab-related conditions, follow therapy progress, attend multidisciplinary meetings, and participate in ongoing education and culture-building activities.

Provide PM&R/physiatry services as an NP/PA rehab consultant at a designated skilled nursing facility. Collaborate with interdisciplinary teams (PT/OT/SLP, PCPs) to manage neurologic and orthopedic conditions, track therapy progress, address barriers to rehabilitation, attend multidisciplinary meetings, and participate in ongoing education and culture-building activities. Work part-time (24–32 hours) with flexible weekday schedule.
