Provides PM&R and physiatry services to patients in skilled nursing and post-acute rehabilitation facilities. Collaborates with physicians, therapy teams, and interdisciplinary staff to manage rehabilitation barriers, including pain, spasticity, neurological conditions, and postoperative recovery. Tracks therapy progress, supports patient independence and quality of life, attends multidisciplinary meetings, and provides clinical leadership and education.

Provide on-site Physical Medicine and Rehabilitation (PM&R) services as a rehab consultant in a skilled nursing facility. Collaborate with interdisciplinary teams (PT/OT/SLP, PCPs) to manage conditions common in post-acute care, track therapy progress, address barriers to therapy, attend multidisciplinary meetings, and support facility staff education. Full-time, 40 hours per week role focused on geriatric post-acute rehabilitation and clinical leadership within the SNF.
