Provide QIDP case management in Community Day Services: develop and implement person-centered plans (PCP/Implementation Strategies), train and supervise CDS staff, conduct assessments, coordinate services with agencies, deliver direct training in daily living, vocational and safety skills, and document progress.

Provide on-site job coaching and vocational training to persons with developmental or mental disabilities, escort them to community training sites, document progress in electronic records, maintain schedules and cleanliness, assist with task analyses and accommodations, and complete required reports and daily time sheets. Must follow IDT and employer recommendations and promote independence and workplace integration.

Coordinate and deliver HR administrative functions including training development and scheduling, onboarding and recruitment support, benefits education, employee surveys and evaluations, HR recordkeeping and reporting, compliance with employment laws, wellness and retention activities, and recommending process improvements to support managers and leadership.
