Provides direct home health and hospice care to frail or elderly patients in Essex County under licensed healthcare professional supervision. Responsibilities include assisting with patient care, using and maintaining medical equipment, maintaining safety, communicating with patients and staff, responding to emergencies, and working independently. Requires a New Jersey Certified Home Health Aide license, driver’s license, auto insurance, safe driving record, medical clearance, immunizations, and at least one year of CHHA experience.

Promotes Journey Hospice to hospitals, physicians, case managers, social workers, healthcare professionals, and community organizations to maintain hospice census. Builds referral relationships, represents the organization in local groups, supports marketing initiatives, and advocates for patients during hospitalization. Covers Chester County as a mobile representative while maintaining professional relationships and timely communication.

Provides in-home care to frail and elderly participants under a registered nurse’s supervision. Follows individualized care plans, assists with health-related daily duties, uses and maintains medical equipment, promotes safety, communicates with patients and care teams, responds to emergencies, and works independently. Requires New Jersey CHHA certification, a driver’s license with insurance, safe driving record, medical clearance, current immunizations, and at least one year of CHHA experience.
