Oversee social services in a long-term care or assisted living facility, manage staff, develop individualized psychosocial care and discharge plans, coordinate community resources, ensure regulatory compliance and documentation, and collaborate with interdisciplinary teams to advocate for resident well-being.

Coordinate and validate MDS assessments for long-term care residents, ensure regulatory compliance, collaborate with interdisciplinary teams to develop care plans, communicate with residents and families, support reimbursement processes, and contribute to quality improvement and audits.
