The Targeted Case Manager provides case management services to individuals with severe mental illness and substance use disorders. Responsibilities include assessing needs, linking clients to resources, conducting advocacy, coordinating care, and ensuring timely documentation of services for 23-25 clients monthly.

Serve as the primary 24-hour access point for individuals in crisis via phone, chat, and text. Perform risk screenings, crisis intervention, de-escalation, safety planning, referrals, scheduling, documentation in EHR, and coordinate care per protocols with supervisory escalation. Participate in training, follow confidentiality standards, and provide shift coverage as needed.

Provide psychiatric services to patients, including evaluations, medication management, and consultations under the supervision of a Licensed Psychiatrist.

Oversee outpatient clinic operations, ensuring compliance and performance. Provide clinical supervision, program coordination, and maintain community relations while delivering direct client services.

Provide psychological services, including assessments, therapy, and supervision of interns. Participate in treatment teams and crisis intervention. Comply with ethical codes and hospital policies.
