Provide case management on a Mobile Crisis Response Team for LCDF CCBHC expansion: develop, evaluate and coordinate treatment and recovery plans, connect clients to resources, maintain confidentiality, use EHR, work collaboratively within interdisciplinary teams, and perform outreach during assigned evening/weekend shifts.

Provide outpatient primary care to patients at a designated La Clinica de Familia site, including diagnosis, treatment, quality assurance participation, cross-coverage, and team leadership. Participate in clinical program improvement and assigned quality activities under the Chief Medical Officer, working within delineation of privileges.

Provide outpatient primary pediatric care at a designated clinic site, participate in quality assurance and clinical program improvements, serve as team leader for site staff, and offer cross-coverage at other locations as needed within scope of practice.
