Provides wraparound case management for children and families in the SED Waiver program. Coordinates treatment teams, referrals, insurance plans, Medicaid-compliant services, documentation, and individualized care plans. Facilitates regular team meetings, monitors outcomes, updates plans, educates partners, and maintains a small caseload. Work occurs across offices, schools, client homes, and other worksites, requiring use of a company vehicle and management of potentially high-stress situations.

Provides wraparound facilitation and case management for children and families in the Serious Emotional Disturbance Waiver Program. Coordinates behavioral health, educational, medical, and community services; facilitates treatment team meetings; develops and monitors individualized plans of care; supports enrollment and transitions; advocates for clients; and completes Medicaid-compliant documentation. The role requires collaboration with families, providers, schools, agencies, and insurers, along with regular community travel and crisis-response capabilities.
