Provides after-hours hospice triage, patient visits, assessments, medications, treatments, care planning, patient education, and clinical documentation. Communicates with patients, families, caregivers, physicians, and staff while evaluating Medicare eligibility and adapting care to changing patient needs. Requires compassionate, collaborative nursing practice and maintenance of hospice clinical competencies.

Provides hospice nursing case management, including patient evaluation and admission, medication and treatment administration, assessments, individualized care plans, nursing interventions, patient education, documentation, physician and family communication, scheduled visits, and participation in interdisciplinary team meetings.

Administer and document resident medications under licensed nurse supervision, provide routine nursing care according to care plans, maintain infection control, protect resident dignity and privacy, and help sustain a clean, safe, warm care environment.

Prepares food trays for general and therapeutic diets, assembles salads and desserts, delivers meals and carts to patient units, maintains clean tray lines and kitchen areas, operates and monitors dishwashers, records wash and rinse temperatures, hand-washes kitchen items, and follows infection-control and safety procedures while treating residents, guests, and coworkers respectfully.

Generate hospice referrals through networking, relationship building, and targeted community marketing. Develop partnerships with healthcare professionals, hospitals, nursing homes, assisted living facilities, and other referral sources. Meet intake and referral goals, collaborate with internal teams, and provide market insights while representing hospice care services compassionately.
