Provide Therapeutic Behavioral Support services to youth with co-occurring behavioral health and developmental disabilities in residential and community settings. Support skill development, symptom management, care coordination, service linkage, and outcome tracking while working with individuals, families, staff, and outside providers.

Provide strengths-based, trauma-informed, recovery-oriented peer support to youth and families within OhioRISE Care Coordination. Collaborate with Child and Family Teams, support engagement, self-advocacy, and individualized care planning using lived experience.

Provide community-based Therapeutic Behavioral Support to youth with co-occurring behavioral health and developmental disabilities. Support skill development, symptom management, service linkage, coordinate with families and providers, and track behavioral health outcomes.

Provide licensed practical nursing services in Intermediate Care Facilities for individuals with intellectual/developmental disabilities. Work within LPN scope under RN supervision, coordinate health needs with care teams, train staff, delegate nursing tasks to certified providers, and ensure compliance with federal, state, and local standards.

Provide community-based case management and service linkage for individuals with developmental and behavioral health needs; coordinate with families, guardians, Boundless staff, and external providers; track health outcomes, assist with referrals, and support whole-person care planning.

Provide clinical decision support for utilization management: monitor service utilization, conduct medical necessity reviews, facilitate payer authorizations, ensure documentation aligns with payer and regulatory expectations, and support compliance, quality, training, and value-based care initiatives.
