Coordinates internal and external healthcare referrals, schedules patient appointments, tracks referral status, gathers clinical documentation, and supports insurance prior authorizations. Serves as a system navigator and patient advocate by addressing healthcare, financial, transportation, interpreter, and social barriers. Maintains accurate registration and charts, communicates with patients and providers, and cross-trains for front desk and medical assistant duties.

Provide individualized nursing care to clients with intellectual and developmental disabilities. Coordinate health needs with client care teams, train team members, and delegate nursing tasks to certified direct service providers while following applicable policies, regulations, and care standards.

Manages and maintains IT systems, networks, workstations, servers, applications, and infrastructure across multiple sites. Provides remote and onsite troubleshooting for Windows, network, operating system, application, Azure, and Active Directory issues. Installs and administers Windows and iOS systems, supports virtualization and cloud environments, and explains technical concepts to nontechnical users. The role requires strong customer service, communication, collaboration, independent judgment, and regular local travel, with occasional overnight travel.
