Provide primary care as a licensed Physician Assistant, ensuring medical services meet legal, licensure, and professional standards. Deliver patient care for individuals with intellectual/developmental disabilities, communicate with medical and community audiences, maintain accurate electronic documentation, and represent the organization. Maintain CPR competency and use computer tools (Microsoft Office). Participate in required pre-employment screening (background, drug, health).

Provide program leadership and direction for Community Integration Services for adults with I/DD and complex behavioral health needs. Oversee day habilitation program operations, supervise staff, coordinate with clinical teams, ensure safe person-centered, community-integrated services, and maintain documentation and compliance.

Provide community-based case management and service linkage for individuals with intellectual/developmental disabilities and behavioral health needs. Coordinate with clients, families, Boundless staff, and external providers to address barriers to care, track health outcomes, and support whole-person wellbeing.
