Directs the home health agency’s clinical services, including staffing, care coordination, quality oversight, policy development, regulatory compliance, documentation review, hiring, staff supervision, education, and patient care delivery. The role requires an RN license, home health experience, supervisory experience, OASIS proficiency, knowledge of Medicare and Medicaid regulations, CPR certification, and a valid driver’s license.

Leads home health growth and strategic partnerships across Broward and Palm Beach Counties. Builds referral relationships with healthcare providers, drives at least 50 Medicare admissions monthly, expands market share, manages territory growth, represents the organization at industry events, and collaborates with operations to support patient outcomes and referral satisfaction. The role requires extensive home health sales leadership experience and a documented record of Medicare census growth.

Provides home health physical therapy services under the supervision of a licensed Physical Therapist. Follows patient care plans, documents treatments and progress, maintains HIPAA confidentiality, submits visit records, attends training, and follows organizational procedures. Serves patients in Fall River and surrounding areas while delivering one-on-one care in their homes.
