Teams at Home Care Providers

Provides skilled nursing care to home-health patients, including assessments, vital-sign monitoring, care-plan implementation, patient and family education, documentation, and coordination with interdisciplinary teams. The role requires independent visits in the Columbus/Dublin, Ohio area while following HIPAA confidentiality and physician-ordered interventions.

Coordinates home health care, conducts SOC, ROC, and recertification visits, supports field clinicians, triages clinical calls, reviews OASIS documentation and care plans, liaises with patients, families, and physicians, and assists with training, evaluations, compliance, staffing, and on-call coverage.

Provides psychosocial evaluations, diagnoses, therapy, referrals, education, and care coordination for home health patients and families. Maintains clinical records, develops treatment plans, leads individual and family sessions, consults with healthcare providers, and supports patients’ social adjustment and recovery while following company, state, federal, and HIPAA requirements.
