Teams at Holisticare Hospice

Provides hands-on hospice nursing care to patients in the community, coordinates interdisciplinary services, assesses hospice eligibility, manages individualized care plans, educates patients and families, advocates for comfort and quality of life, responds to clinical referrals, and participates in call duties.

Build and maintain relationships with physicians, healthcare partners, discharge planners, skilled nursing facilities, social workers, and community agencies. Promote hospice services, generate referrals, support census growth, and position the organization as a preferred provider. Lead initial hospice-care conversations with patients and families, explain services and philosophy, and facilitate consent compassionately across the Southern Ocean County territory.

Build relationships with physicians, healthcare partners, hospitals, skilled nursing facilities, social workers, and community agencies to generate hospice referrals. Promote hospice services, support census growth, conduct outreach, explain hospice care to patients and families, and facilitate consent conversations. The role requires healthcare sales or marketing experience, strong relationship-building skills, reliable transportation, a valid driver’s license, and auto insurance.
