Teams at Holisticare Hospice

Provides hospice nursing and case management for patients and families. Responsibilities include assessing hospice eligibility, delivering direct comfort care, coordinating individualized care plans, educating patients and caregivers, collaborating with clinical staff, responding to referrals, and participating in call duties. The role requires a current Registered Nurse license, at least one year of acute or medical-surgical nursing experience, computer and EMR proficiency, and CPR certification where required.

Provides per diem hospice nursing care across Lehigh and Northampton counties. Responsibilities include assessing hospice eligibility, delivering hands-on end-of-life care, coordinating interdisciplinary services, educating patients and families, developing personalized care plans, supporting comfort and quality of life, responding to referrals, and participating in on-call triage. Requires a current RN license, acute or medical-surgical nursing experience, EMR proficiency, CPR certification when required, and dependable transportation.
