Provides skilled hospice, palliative, and end-of-life nursing care in community settings. Coordinates interdisciplinary care plans, medical management, and hospice services for patients and families, while promoting quality of life and supporting loved ones. Requires weekday availability with an on-call rotation and use of a personal vehicle for patient visits.

Provides after-hours hospice nursing care through patient and family phone support, home visits, admissions, comprehensive and PRN visits, imminent-death and death visits, and tuck-in visits. Collaborates with the interdisciplinary team and responds to patients’ end-of-life care needs during evening, overnight, weekend, and holiday coverage.

Prepares and submits hospice claims, statements, and invoices to Medi-Cal, Medicare, and private insurers. Verifies insurance coverage, obtains authorizations, resolves and resubmits denied claims, manages accounts receivable, submits appeals, monitors electronic billing, and follows up on outstanding payments. The specialist also maintains compliance with changing healthcare billing regulations and collaborates with billing, administrative, and clinical staff.

Provides skilled palliative and end-of-life care to hospice patients in community settings. Coordinates interdisciplinary care plans, medical management, and hospice services while supporting patients, families, and significant others. Promotes hospice philosophy and quality-of-life care. Requires rotating weekend availability, community travel using a personal vehicle, and the ability to perform physical tasks such as lifting up to 35 pounds.

Provides pediatric hospice and concurrent care case management for terminally ill patients and their families across Fresno, Madera, and Merced counties. Promotes hospice philosophy, coordinates interdisciplinary care, manages complex end-of-life situations, maintains clinical documentation and confidentiality, and supports patients and loved ones with compassionate, quality-focused care.

Provide executive leadership for all clinical programs and services, overseeing clinical operations, quality improvement, patient safety, education, and outcomes across hospice, palliative, pediatric, bereavement, and community grief services. Lead strategy, regulatory compliance, accreditation readiness, and interdisciplinary team development to ensure mission-aligned, high-quality care delivery.
