Coordinates healthcare quality-improvement initiatives and Aledade value-based care activities. Analyzes and validates clinical and operational data, monitors HEDIS and other quality measures, manages dashboards and worklists, coordinates patient outreach and care-gap follow-up, develops workflows and training materials, prepares reports and meeting documentation, and collaborates with clinical and administrative teams to improve outcomes and sustain performance.

Provide comprehensive primary care to patients of all ages: diagnose and manage acute and chronic conditions, create treatment plans, document in EHR, coordinate multidisciplinary care, participate in quality improvement, mentor advanced practice providers, and support community outreach and FQHC regulatory compliance.

Provide comprehensive primary pediatric care for patients birth through adolescence, including well-child visits, immunizations, acute and chronic disease management, developmental screening, counseling, care coordination, EHR documentation, quality improvement, and supervision/mentorship in an FQHC setting.

Provides case management for individuals in Medication for Addiction Treatment and Enhanced Care Management, coordinating care across medical and behavioral providers, conducting outreach and home visits, developing care plans, addressing social needs, documenting in the EHR, and meeting ECM/MAT productivity and billing requirements.
