Coordinates healthcare quality-improvement initiatives and Aledade value-based care activities. Analyzes and validates clinical and operational data, monitors HEDIS and other quality measures, manages dashboards and worklists, coordinates patient outreach and care-gap follow-up, develops workflows and training materials, prepares reports and meeting documentation, and collaborates with clinical and administrative teams to improve outcomes and sustain performance.

Provide comprehensive primary care to patients of all ages: diagnose and manage acute and chronic conditions, create treatment plans, document in EHR, coordinate multidisciplinary care, participate in quality improvement, mentor advanced practice providers, and support community outreach and FQHC regulatory compliance.

Manage and implement a community wellness program: develop protocols and budgets, coordinate outreach and partnerships, supervise staff and volunteers, collect/report data, ensure regulatory compliance, and oversee property and vehicle coordination for program delivery.
