Nurse Supervisor manages and coaches Care Navigators delivering chronic care management and remote patient monitoring. Responsibilities include assigning caseloads, supporting patient enrollment, monitoring performance, conducting quality reviews, providing workflow training and corrective coaching, leading team meetings, and ensuring compliance with clinical, legal, and partner requirements. The role collaborates with Clinical Operations and Customer Success teams to improve patient outcomes, navigator engagement, and partner satisfaction.

Enroll patients in chronic care management and remote patient monitoring programs through outbound calls. Explain program benefits, costs, consent, and device shipment; manage enrollment caseloads; enter information across systems; provide empathetic patient support; and meet monthly enrollment goals.

Own a portfolio of health system and multi-location practice partners as their senior advisor. Develop tailored success plans, drive executive business reviews, quantify ROI, resolve escalations, optimize billing and payer strategy, identify expansions, mentor junior CSMs, and collaborate cross-functionally to improve product and processes.

Serve as primary customer contact for private practice clients, guiding implementation through renewal. Monitor clinical and billing performance, lead business reviews, diagnose adoption or reimbursement issues, develop action plans, educate on Medicare/RPM billing, and collaborate cross-functionally to improve enrollment, utilization, reimbursement, and retention.
