Coordinates safe, timely hospital discharges and transitions to community or institutional care. Develops and documents care plans, identifies discharge barriers, arranges referrals, educates patients and families, coordinates legal and substitute decision-maker issues, and collaborates with interdisciplinary teams and community agencies. The role also supports patient flow, resource utilization, complex case management, and advocacy for service gaps.

Provides specialized assessment, treatment, and neurorehabilitation services to adults with neurological conditions in an outpatient program. Supports functional goals and independence, with emphasis on stroke care and neurological rehabilitation. Collaborates with occupational therapists, speech-language pathologists, social workers, and OTA/PTAs to deliver comprehensive patient care. Works independently and as part of a multidisciplinary team across scheduled shifts.

Provides physiotherapy assessment and treatment for musculoskeletal, geriatric, neurological, and orthopedic conditions. Supports the WSIB Specialty Program through interdisciplinary assessment, rehabilitation, return-to-work planning, functional evaluation, and work conditioning. Responsibilities also include patient education, community promotion, program development, accreditation support, and daily program operations while delivering client-centered care and collaborating with multidisciplinary teams.

Coordinates hospital staffing requests, uses scheduling software to identify and offer available shifts, updates electronic schedules, communicates with staff and units, deploys nursing resource personnel according to procedures and collective agreements, and performs clerical duties.

Leads hospital-based clinical research operations, including investigator-initiated studies, clinical trials, quality improvement initiatives, research infrastructure, regulatory and ethics compliance, grants, contracts, budgets, financial tracking, and institutional partnerships. Supports physicians and clinical teams, develops policies and workflows, oversees research agreements and funding administration, and contributes to research strategy and program growth.
