Provide exercise assessments, personal training, and group classes for residents; monitor vitals, respond to emergencies, counsel on lifestyle change, and complete administrative tasks to support facility goals.

Support sales team by managing leads, scheduling, waitlists, and resident intake; coordinate marketing initiatives and agency liaison; ensure brand quality of print and digital materials; prepare reports using Salesforce, Pardot, and spreadsheets; assist special projects and enhance resident experience.

Provide direct nursing care and supervise nursing assistants across care levels (SNF, Assisted Living, Dementia, Independent Living). Administer medications, perform treatments, manage care plans, respond to resident needs, assist with ADLs, participate in admissions and care plan meetings, and report critical issues to nursing leadership.

Provide psychosocial support and adjustment counseling to residents and families, perform social work assessments, complete assigned MDS sections, develop and update care plans, document progress notes in PCC, coordinate admissions/discharges, participate in interdisciplinary teams, communicate with healthcare providers, assist with transitions of care, crisis intervention, and facilitate family/resident care plan meetings.
