Licensed Practical Nurse providing hands-on care across skilled nursing, assisted living, dementia care, and independent living settings. Responsibilities include medication administration, treatments, specimen collection, resident monitoring, documentation, emergency response, triage, and reporting. The role also serves as Charge Nurse, providing team oversight, leadership, and administrative support during 12-hour weekend shifts.

Provide exercise assessments, personal training, and group classes for residents; monitor vitals, respond to emergencies, counsel on lifestyle change, and complete administrative tasks to support facility goals.

Provide direct nursing care and supervise nursing assistants across care levels (SNF, Assisted Living, Dementia, Independent Living). Administer medications, perform treatments, manage care plans, respond to resident needs, assist with ADLs, participate in admissions and care plan meetings, and report critical issues to nursing leadership.
