Provide skilled, high-acuity home health nursing with focus on wound care and infusion therapy. Perform assessments and documentation in HCHB, coordinate care with physicians and therapists, manage casework, administer treatments, supervise LVNs/HHAs, and support quality and safety initiatives.

Serve as the on-site bridge between facilities and home health operations: build relationships with discharge planners, identify and convert referrals, conduct bedside visits, coordinate intake/EMR documentation, expedite insurance and scheduling issues, and provide clinical education and follow-up to ensure smooth transitions to home health services.

Lead clinical operations for a home health caseload: review referrals, coordinate care planning, supervise field staff, ensure OASIS/compliance, support utilization reviews, participate in call rotation, and manage documentation and interdisciplinary communication.

Own a defined home health sales territory, manage 20–30 key accounts, drive referral and admission growth, and coach 2–5 sales representatives. Responsibilities include territory planning, facility relationship development, pipeline reviews, performance tracking, recruiting support, market strategy execution, and reporting results to the Regional Sales Manager. The role is a player-coach position with a path to Regional Sales Manager.

Serve as the on-site clinical bridge between facilities and home health operations: build relationships with discharge teams, identify and convert referrals, conduct bedside patient education, coordinate intake/insurance, track referrals to start-of-care, and escalate clinical issues to intake or clinical teams.

Lead branch clinical execution and quality for home health: provide direct clinical guidance, oversee staff development and skills validation, manage care coordination, prevent rehospitalizations, ensure compliance and survey readiness, and act as escalation leader for high-risk patients.

Serve as on-site bridge between facilities and home health: build relationships with case managers, identify appropriate patients, conduct bedside visits, ensure complete referrals and timely intake, coordinate with intake and account teams, provide clinical education and escalate issues to support smooth transitions to home health.

As a Physical Therapist Assistant, you will deliver skilled care at home, execute treatment plans, document progress, and coach patients on practical programs while collaborating with a supervising PT.
