As a Physical Therapist Assistant, you will deliver skilled care at home, execute treatment plans, document progress, and coach patients on practical programs while collaborating with a supervising PT.

Provide skilled, high-acuity home health nursing with focus on wound care and infusion therapy. Perform assessments and documentation in HCHB, coordinate care with physicians and therapists, manage casework, administer treatments, supervise LVNs/HHAs, and support quality and safety initiatives.

Lead and manage a home-based orthopedic program, develop clinical protocols, oversee interdisciplinary care, ensure regulatory compliance, monitor outcomes and quality metrics, train staff, coordinate with referral sources and hospitals, and drive program growth and continuous improvement.

Serve as the on-site bridge between facilities and home health teams to generate and convert referrals, conduct bedside patient education, coordinate intake/insurance, maintain facility relationships, and ensure smooth transitions to home health services.

Own and grow a defined territory as a player-coach: manage a personal book of accounts, coach 2–5 sales reps, build facility relationships with discharge planners and case managers, run weekly pipeline and performance cadences, and drive quarter-over-quarter admission growth using data-driven territory plans.

Provide skilled home health nursing including comprehensive in-home assessments, wound care and infusion therapy, medication administration, care planning, documentation in Homecare Homebase, coordination with physicians and caregivers, and supervision of LVNs and HHAs. Participate in case conferences and in-services, travel between patient homes, and meet productivity targets.

Coordinate home health referrals for assigned Account Executives by obtaining discharge dates, orders, and paperwork from hospitals and EMR portals, calling patients to schedule start-of-care, monitoring pending referrals, ensuring HIPAA-compliant documentation, and escalating issues to prevent admission delays.

Lead branch clinical execution and quality for home health: provide direct clinical guidance, oversee staff development and skills validation, manage care coordination, prevent rehospitalizations, ensure compliance and survey readiness, and act as escalation leader for high-risk patients.

Serve as on-site bridge between facilities and home health: build relationships with case managers, identify appropriate patients, conduct bedside visits, ensure complete referrals and timely intake, coordinate with intake and account teams, provide clinical education and escalate issues to support smooth transitions to home health.
