Provides skilled home health nursing in patients’ homes, including assessments, wound care, infusion therapy, medication administration, disease-management education, documentation, care-plan development, and care coordination. Supervises LVNs and HHAs, communicates with physicians and families, participates in case conferences, and performs physician-ordered procedures within scope. Requires a Texas RN license, clinical nursing experience, CPR/BLS certification, a driver’s license, and home health software competency.

Provides skilled, high-acuity nursing care in patients’ homes, focusing on wound care and infusion therapy. Conducts assessments and OASIS documentation, develops and coordinates care plans, administers treatments, educates patients and caregivers, communicates with providers, resolves care barriers, and supervises LVNs and HHAs. The role requires autonomous field-based case management, care coordination, and continuity of care across multidisciplinary teams.

Serve as the on-site clinical bridge between facilities and home health operations: build relationships with discharge teams, identify and convert referrals, conduct bedside patient education, coordinate intake/insurance, track referrals to start-of-care, and escalate clinical issues to intake or clinical teams.
