Provide skilled home health nursing including comprehensive in-home assessments, wound care and infusion therapy, medication administration, care planning, documentation in Homecare Homebase, coordination with physicians and caregivers, and supervision of LVNs and HHAs. Participate in case conferences and in-services, travel between patient homes, and meet productivity targets.

Provide comprehensive in-home nursing care including wound care, infusion therapy, medication administration, skilled documentation, patient advocacy, care coordination, teaching, and participation in case conferences. Requires Texas LPN license and three years clinical experience.

Lead clinical operations for a home health caseload: review referrals, coordinate care planning, supervise field staff, ensure OASIS/compliance, support utilization reviews, participate in call rotation, and manage documentation and interdisciplinary communication.
