Provides outreach and intensive case management to 25–30 individuals experiencing homelessness, substance use, behavioral health conditions, and criminal legal system involvement. Responsibilities include participant engagement, needs assessments, individualized service planning, housing and benefits assistance, advocacy, court support, referrals, provider coordination, recordkeeping, and collaboration with LEAD partners and community organizations. The role also supports harm reduction, crisis de-escalation, resource access, and systemic equity initiatives.

Conduct substance use disorder assessments, develop treatment and safety plans, provide individual and group counseling, manage cases, coordinate patient care, and connect patients with community resources. Maintain accurate clinical documentation, confidentiality, and regulatory compliance. Collaborate with medical, psychiatric, and community teams, participate in treatment reviews, and support patients across clinic, inpatient, and community settings. Local agency travel may be required.

Supervise LEAD case managers and outreach coordinators, provide clinical and administrative oversight, support program expansion, ensure compliance with agency/contract requirements, coordinate with law enforcement and community partners, conduct performance evaluations, manage staffing coverage and client records, and represent the program to stakeholders.
