Coordinate and oversee individualized programs for members with intellectual, developmental, and psychiatric disabilities. Supervise and train staff, support person-centered planning and implementation, ensure compliance with funding and reporting requirements, maintain member records, maximize community access, respond to medical/behavioral emergencies, assist with benefits/placement, and manage billing and attendance reporting.

Use lived experience to mentor, assist with treatment and crisis plans, connect clients to housing/benefits/resources, support independent-living skills, perform crisis intervention and de-escalation, document interactions, and meet productivity goals.

Provide advocacy, care coordination, and person-centered planning for adults with developmental disabilities. Build relationships with clients and families, manage service agreements and referrals, maintain electronic records (Quantum), partner with community resources, support during crises, and perform frequent community-based visits across Cook County.

Provide community-based psychiatric nursing for adults with serious mental illness: coordinate medical care and annual evaluations, administer injectable psychotropic medications, monitor labs/vitals/medications, conduct home visits and emergency response, participate in treatment planning and documentation (Medicaid Rule 132/140), coordinate with providers, and provide education and support.
