Technology-enabled service for family caregivers of chronically ill populations
eFamilyCare supports family caregivers – there are over 50 million in the country, and while family members provide over 80% of caregiving for chronic illness, they are without any support. We are unique in providing a dedicated care adviser via a proprietary digital app that offers advice on how to cope with both medical and non-medical problems, as well as emotional support to the often-isolated family caregiver. eFamilyCare contracts with health plans, employers and hospitals who pay for the service because it improves the quality of care, addresses Social Determinants of Health through customized support and referrals, and reducing the cost of unnecessary hospitalizations and emergency room visits. Each member of the eFamilyCare management team has 20+ years of experience with providing healthcare to chronically ill patients (Humana, NYU Hospitals Center system, SeniorBridge, Health Dialog, Helius Medical).
