Provides weekend on-call hospice nursing care across Orangeburg and surrounding counties. Responds to urgent and routine patient needs, conducts comprehensive physical, emotional, social, and spiritual assessments, documents care accurately in home and agency systems, coordinates services, and educates and supports families caring for terminally ill loved ones.

Provides hospice nursing care throughout the community, coordinates interdisciplinary services, assesses hospice eligibility, manages patient care plans, delivers direct clinical care, educates patients and families, advocates for comfort and quality of life, and participates in call duties. Requires a current Registered Nurse license, at least one year of RN experience, EMR proficiency, CPR certification when required, and dependable transportation.

Provides weekend on-call hospice nursing care for patients and families across the TriCounty territory. Responsibilities include responding to urgent and routine needs, conducting comprehensive physical and psychosocial assessments, coordinating care, documenting accurately in patient homes and agency systems, and educating families caring for terminally ill loved ones.

Build and maintain relationships with physicians, healthcare professionals, referral sources, and community agencies to grow hospice referrals and census. Conduct outreach, promote hospice services, explain care options to patients and families, and facilitate hospice consent conversations with empathy. Manage a designated regional territory while representing the organization’s mission and coordinating with clinical and marketing teams.

Provides on-call hospice nursing care on a seven-days-on, seven-days-off schedule. Responds to urgent and routine patient needs, conducts comprehensive physical and psychosocial assessments, documents care accurately, coordinates services, and supports families caring for terminally ill loved ones. Requires an unencumbered RN license, at least one year of medical-surgical or acute-care experience, computer and EMR proficiency, and current CPR certification when required.

Provides hospice nursing care throughout a community territory, including direct patient care, eligibility assessments, care coordination, symptom management, patient and family education, personalized care planning, and interdisciplinary collaboration. The RN Case Manager responds to referrals, supports comfort and quality of life, maintains clinical documentation, and participates in call duties while guiding patients and families through end-of-life care.

Leads hospice clinical operations and interdisciplinary care teams, coordinates patient services, audits medical records for regulatory compliance, develops staff training, evaluates clinical performance, and facilitates care-plan meetings. The role requires an active RN license, hospice or related direct-care experience, supervisory experience, clinical expertise in terminal care, computer and EMR proficiency, CPR certification when required, and dependable transportation.

Provides direct hospice nursing care throughout the community, manages patient cases, assesses hospice eligibility, coordinates interdisciplinary services, administers comfort-focused care, educates patients and families, develops personalized care plans, and participates in call duties.
