Teams at Crescent Hospice

Promotes hospice services throughout the community by building relationships with physicians, healthcare partners, discharge planners, skilled nursing facilities, social workers, and referral sources. Drives referral and census growth through outreach, explains hospice services and philosophy to patients and families, and facilitates compassionate conversations and consent. Requires healthcare sales or marketing experience, strong relationship-building skills, reliable transportation, a valid driver's license, and auto insurance.

Provides hospice nursing care to patients and families throughout the community. Responsibilities include assessing hospice eligibility, coordinating interdisciplinary services, delivering direct comfort-focused care, educating patients and caregivers, developing personalized care plans, responding to clinical referrals, and participating in call duties. Requires a current Registered Nurse license, at least one year of medical-surgical or acute-care nursing experience, CPR certification, EMR proficiency, and dependable transportation.
