Teams at Crescent Hospice

Provides hands-on hospice nursing care, coordinates services, assesses hospice eligibility, educates patients and families, develops individualized care plans, and supports comfort and quality of life. Collaborates with the clinical director and interdisciplinary team while participating in call duties. Serves patients across Berkeley, Charleston, and Dorchester counties on a Monday-Friday schedule.

Provides weekend on-call hospice nursing care across Orangeburg and surrounding counties. Responds to urgent and routine patient needs, conducts comprehensive physical, emotional, social, and spiritual assessments, documents care accurately in home and agency systems, coordinates services, and educates and supports families caring for terminally ill loved ones.

Provides hospice nursing care throughout the community, coordinates interdisciplinary services, assesses hospice eligibility, manages patient care plans, delivers direct clinical care, educates patients and families, advocates for comfort and quality of life, and participates in call duties. Requires a current Registered Nurse license, at least one year of RN experience, EMR proficiency, CPR certification when required, and dependable transportation.
