Teams at Contra Costa Interfaith Housing

Provides home-based case management and supportive mental health services to formerly homeless families. Conducts psychosocial, life skills, and financial assessments; develops family action plans; provides crisis and behavioral support; coordinates medical, mental health, addiction, legal, and community services; conducts home visits; documents services and maintains HMIS data; participates in case conferences, supervision, on-call coverage, and community partner coordination.
