Oversee the Buffalo Trace Point of Entry early intervention program, including staff hiring, caseload monitoring, training, quality assurance, documentation, reporting, contract compliance, referrals, community outreach, and stakeholder relationships. Facilitate staff and council meetings, represent the program at community events, and provide early intervention training to community partners. The role may include home visits and requires dependable transportation.

The Community Support Associate assists youth in engaging with community activities, enforces behavior plans, and coordinates with an interdisciplinary team. They manage treatment plans, complete training, and maintain documentation while providing a safe and supportive environment.

Supervise participants and staff in a therapeutic rehabilitation program, providing direct services, coordination of care, and clinical oversight to support adult SMI population towards wellness.

Provides counseling, crisis intervention, client assessment, service coordination, psychoeducation, and follow-up care for individuals and families. Coordinates 988 and crisis team operations, including staffing, scheduling, documentation compliance, supervision, training, quality assurance, and communication with community partners. Requires flexible scheduling, on-call participation, community crisis response, and travel. The role maintains a reduced clinical caseload while supporting program performance and continuity of care.

Provide supervision and personal assistance per a participant's person-centered plan to promote independence and community integration. Duties include ADLs, household tasks, medication management and documentation, appointment scheduling, paperwork, communication with family and staff, socialization support, money management, shopping, and facilitating community participation.
