Front-desk receptionist for a skilled nursing facility responsible for greeting visitors, answering and routing phones, providing information and receipts, assisting clerical and business office tasks, ordering supplies, and supporting administrative functions. Must maintain a professional image and communicate with residents, families, staff, and agencies.

Provide administrative and client support to social workers and case managers for residents in a skilled nursing facility. Conduct admission interviews, inform residents and families of rights, coordinate care services, advocate with social service organizations, maintain records, ensure timely communication and follow-up, and assist residents adjusting to facility or community living.

The Dietary Aide assists in meal preparation and serving, maintains cleanliness in the kitchen, and follows safety regulations to ensure a safe dining environment for residents.

Provide nutrition assessments and individualized care plans for skilled nursing residents; monitor nutritional status and weight; collaborate with physicians, nursing, and interdisciplinary teams; participate in care conferences, QAPI, and regulatory compliance; support food service operations and staff education; float between multiple facilities.

The role involves providing direct care to residents, assisting with medication, daily activities, and ensuring a respectful, nurturing environment for seniors.

The MDS Nurse oversees the MDS/RAI process, reviews nursing documentation, implements care plans, and ensures timely transmission of MDS packages.
