Provides community-based case management, crisis support, and goal-directed interventions for children with severe emotional disturbances and their families. Maintains treatment-plan contacts, completes progress documentation, attends treatment-plan and crisis meetings, coordinates assessments, and keeps scheduling records current. Requires flexible hours, strong organization and teamwork, mental health or human services education or equivalent experience, required training completion, and a valid driver’s license.

Provides individualized peer support to adults with mental health or addiction issues. Uses lived recovery experience to help clients establish personal recovery goals, build stability, make decisions, and access community resources. Collaborates with interdisciplinary treatment teams and supports clients and families in the community. Requires lived experience with mental health or addiction services, at least one year in recovery, required state training, CPR and First Aid certification, and a valid driver’s license.

Provides community-based psychiatric support and treatment to children, adolescents, and families through goal-directed interventions tied to individualized treatment plans. Services are delivered face-to-face in agency, school, home, and community settings. The role requires collaboration with families and other support individuals, flexibility in changing community-based programs, case management certification, and completion of required safety and service trainings.

Provides individualized peer support to adults experiencing mental health or addiction issues. Helps clients establish recovery goals, promotes self-determination, connects clients with community resources, and participates on an interdisciplinary treatment team. Requires lived experience with mental health or addiction services and at least one year in recovery. The employer will train qualified candidates and requires state certification, CPR, First Aid, and a valid driver's license.

Provides face-to-face community psychiatric support and treatment to children, adolescents, and families. Delivers goal-directed supports and solution-focused interventions in agency, school, home, and community settings according to individualized treatment plans. Coordinates with families and collateral contacts, adapts to changing community-based service needs, completes required certifications, and maintains administrative accountability to the program supervisor.

Provides targeted case management and wraparound facilitation for children, adolescents, and families. Coordinates medical, social, educational, and community services; develops wraparound teams and plans of care; monitors treatment progress; completes waiver paperwork and documentation; and updates services as needed. Requires collaboration with consumers, families, and service providers, along with required state, CPR, first aid, CPI, and community event certifications.

Processes and tracks compliance data using Excel, reviews documents for accuracy, obtains required records, and coordinates with internal staff, state agencies, managed care organizations, and foster care agencies. The role requires strong communication, organization, attention to detail, Microsoft Office skills, and occasional automobile travel. A high school diploma is required; office or healthcare experience is preferred.
