Teams at Community Health Alliance

Provide community-based case management to individuals with mental illness: coordinate assessments and treatment planning, facilitate access to healthcare and social services, teach daily living skills, monitor symptoms, assist in crisis management and stabilization, link clients to community resources and support groups, perform home visits, and collaborate with treatment teams, families, and providers.

Provide clinical supervision, training, and oversight for outpatient/community mental health clinicians. Deliver direct therapy, ensure documentation and regulatory compliance, support hiring/onboarding, lead quality improvement, and coordinate care with partners to improve client outcomes.

Greet clients and provide compassionate front-desk support for behavioral health programs. Manage calls, scheduling, patient registration, electronic health records, waitlists, and office operations. Ensure HIPAA and 42 CFR Part 2 compliance while coordinating with clinical staff and supporting administrative reports and projects.
