Supports oversight of a Long-Term Structured Residence behavioral health program through staff supervision, clinical and administrative coordination, regulatory compliance, admissions and discharge oversight, training, safety management, documentation, and emergency response. The role also coordinates with external agencies, supports payroll oversight, maintains therapeutic operations, and provides 24-hour on-call coverage.

Provides bilingual, community-based case management for adults by assessing needs, developing service plans, coordinating resources, monitoring health and provider engagement, maintaining documentation, advocating for services, and supporting crisis management and independence. Services are delivered in clients’ homes, hospitals, and community settings.

Provides community-based case management for adults, including young adults and Hispanic community members. Assesses needs, develops service plans, coordinates behavioral health and community resources, monitors client progress, maintains documentation, supports crisis management and de-escalation, advocates for services, and promotes client stability, independence, and successful navigation of care systems.

Coordinates and monitors Individual Support Plans for people with intellectual, developmental, and behavioral health needs. Conducts assessments, participates in ISP meetings, trains direct support staff, reviews documentation, coordinates services, monitors incidents, maintains records, and supports behavioral and life-skills goals. Ensures compliance with Pennsylvania regulations, promotes independence and community integration, and collaborates with consumers, families, and interdisciplinary teams.

Provides individualized employment support to adults with disabilities through job development, on-the-job training, counseling, assessments, and community-based employment opportunities. Collaborates with employers, families, case managers, funding agencies, and internal teams to promote independence, paid employment, community integration, and long-term job success. Maintains service documentation, follows program requirements, and participates in professional development. The role requires communication skills, flexibility, a valid driver’s license, access to a vehicle, and occasional lifting.

Provides psychiatric evaluations, diagnoses, treatment planning, medication management, crisis intervention, and ongoing patient monitoring for individuals with mental health, behavioral, emotional, or developmental needs. Collaborates with therapists, nurses, and case managers, documents care in electronic medical records, maintains confidentiality, responds to patient and family inquiries, and supports scheduling and billing processes in an outpatient mental health setting.

Register patients in the electronic medical record, verify insurance eligibility and authorizations, collect payments and documentation, maintain accurate demographic and insurance records, and support patient flow. The role communicates registration requirements to patients and families, collaborates with clinical and administrative teams, resolves registration issues, and follows HIPAA, privacy, and organizational compliance standards. Bilingual English and Spanish communication is required.
