Coordinate community-based screenings and referrals using the Integrated Face Sheet and SCAN Tool. Conduct on-site engagement and assessments, send referrals, track SCAN results, collaborate with partners and service providers, and support members with social determinants of health needs. Maintain documentation and comply with relevant policies and quality standards.

Lead day-to-day accounting and financial operations, prepare financial statements and budget variance reports, manage multi-state payroll, supervise A/P and A/R, ensure CFR and audit compliance, mentor finance staff, support CFO and Board reporting, and oversee restricted grants and government contract tracking.

Perform outreach and engagement to enroll high-risk adults and children into Health Home Care Management services. Complete required enrollment documentation and assessments, distribute basic needs, make referrals, and coordinate care with interdisciplinary teams and community partners. May provide ongoing care management, create plans of care, and maintain records in the agency HIT system.
