Coordinate community-based screenings and referrals using the Integrated Face Sheet and SCAN Tool. Conduct on-site engagement and assessments, send referrals, track SCAN results, collaborate with partners and service providers, and support members with social determinants of health needs. Maintain documentation and comply with relevant policies and quality standards.

Lead day-to-day accounting and financial operations, prepare financial statements and budget variance reports, manage multi-state payroll, supervise A/P and A/R, ensure CFR and audit compliance, mentor finance staff, support CFO and Board reporting, and oversee restricted grants and government contract tracking.

The CCS Site Coordinator facilitates connections between school and community resources, oversees student support services, and manages program activities to enhance student well-being and engagement.
